  1. Click USYS Medical Release button to download and complete the Medical Release form
  2. Click the "Register" button and complete the PlayMetrics registration.  Be sure to upload the Medical Release. 
  3. Acknowledge through email confirmation.
  4. Done!  We look forward to seeing you!
